  46. 1. INTRODUCTION

  48. With Medical Consultant for Windows, you can generate a personal medical 
  49. history; write a living will; and create a durable power of attorney.  Medical 
  50. histories can be saved for future reference, updated as necessary, and printed in 
  51. either pocket or full-size format.
  52.  
  53. For details about the use of a mouse, and the use of Windows software please 
  54. see the Windows 3.0 manual.  The minimum system requirements for Windows 
  55. 3.0 and Medical Consultant are: 2 Mb RAM, an 80286 CPU and a hard disk drive.  
  56. A VGA monitor is strongly recommended.

  58. Legal stuff:

  60. The program was written by Mark Ebell MD, a board-certified family physician.  
  61. Neither Dr. Ebell nor Doggone Software Ltd. are responsible for the results 
  62. (good or bad) based on histories taken using this software.  It is in no way 
  63. guaranteed to be complete or accurate, and should of course be supplemented by
  64. a careful oral history.

  66. Similarly, the Living Wills and Durable Powers of Attorney created using this 
  67. document should be reviewed by an attorney to ensure that they meet 
  68. specific requirements for your jurisdiction. They are not guaranteed to be legal
  69. in all 50 states or in every jurisdiction.

  72. WHY YOU SHOULD DEFINITELY REGISTER THIS PROGRAM:

  74. Registered users get the latest version of the program on diskette, a clear 
  75. conscience, and the knowledge that they are supporting the development of 
  76. more great medical software.  Plus, the registered version doesn't have those 
  77. annoying nag screens, and prints reports without the "Unregistered..." label at the 
  78. top.

  80. As if that isn't enough, registered users also get a free bonus program, the 
  81. Cardiovascular Risk Analyst for Windows.  This program calculates your risk of 
  82. developing heart disease and/ or stroke in the next five to ten years and prints a 
  83. detailed textual risk factor analysis. It is available only to registered users of 
  84. Doggone Software products.

  94. 2. INSTALLATION / RUNNING THE PROGRAM

  96. To install the Medical Consultant program:
  97.  
  98. Just copy the CONSULT.EXE file to any directory on your hard drive.  The 
  99. program consists of the single file, and can be run from a floppy diskette.  
  100.  
  101. Since the program was written with the VisualBasic programming language, 
  102. the VBRUN100.DLL file must be present in your path.  It is recommended that 
  103. it reside in the \WINDOWS directory.  The file can be downloaded from the 
  104. MSBASIC forum library on CompuServe, as well as from numerous bulletin 
  105. board services.  It is included for your convenience on the Medical  Consultant 
  106. distribution diskette.

  109. To run the program:
  110.  
  111. Run the CONSULT.EXE file from the Program Manager or File Manager.  The 
  112. program takes 5-10 seconds to load because the large VBRUN100.DLL file 
  113. must also be loaded into your computer's memory.  
  114.  
  115. The program essentially consists of a series of forms asking you for 
  116. information, which the program organizes into documents and reports.  To 
  117. move around within a form, use the Tab key to move forward from box to box.  
  118. To move backward, just hold the Shift key while pressing Tab.
  119.  
  120. Help is usually just the press of a button away, and you can always quit what 
  121. you are doing by pressing the Cancel button.

  126. 3. PERSONAL MEDICAL HISTORY
  127.  
  128. A detailed personal medical history can be very useful for most people.  In today's 
  129. mobile society, it is the rare patient who sees one physician their entire life.  More 
  130. likely than not, you will move several times in your life, meet many physicians, and 
  131. at times seek aid in emergency rooms and urgent care centers.
  132.  
  133. At such times, it is important that your treating physician have a detailed medical 
  134. history.  A detail which may seem unimportant or irrelevant to you can help the 
  135. physician make their diagnosis, and prevent unnecessary, expensive and 
  136. potentially dangerous testing.

  139. To begin a new history:
  140.  
  141. Select File - New history (i.e., open the File menu, then select New history...).  
  142. You will be asked if you would like to create a new history... press the Yes 
  143. button.  Next, a form will appear, and you will be asked to enter your name, 
  144. age and gender.   
  145.  
  146. A complete medical history consists of a series of sub-histories, such as a 
  147. history of medications, a history of allergies, and so on.  To complete your 
  148. personal medical history, select the sub-histories from the History menu one at 
  149. a time, and fill them out.  Take your time, and answer the questions as 
  150. accurately and completely as possible.  After you have answered all of the 
  151. questions in a sub-history which you care to answer, press the Done button.  
  152. A check will appear next to the name of the sub-history in the main menu, 
  153. indicating that it has been changed or updated during this session.  Then, go 
  154. on to a sub-history which hasn t been filled out yet.

  156. When you have answered all of the questions in all of the sub-histories, select 
  157. Save as... from the File menu.  You will be given the opportunity to name your 
  158. history.  Choose a name in the format:  xxxxxxxx.HST.  Examples would 
  159. include MARK.HST or CLARA2.HST.

  162. To edit a previous history:
  163.  
  164. Select File - Open history.  Then, just select the appropriate *.HST file from the 
  165. hard drive or a floppy, and it will be loaded.  Finally, select the appropriate sub-
  166. history from the History menu, and update the information.

  169. To print the currently loaded file:
  170.  
  171. Select File - Print...  You will be given the option of printing a full-size or pocket 
  172. size history.  Select your preference, then press the OK button when asked 
  173. whether or not you would like to print.  A laser printer is recommended for the 
  174. pocket-size history because of the size of the type face used.

  179. 4. HEALTH CARE TREATMENT DIRECTIVE (LIVING WILL) 
  180.  
  181. The Living Will is a document which allows you to state your preferences 
  182. regarding the kind of medical care you prefer when you can no longer participate 
  183. in decisions about your care.  It is a very useful document, and widely recognized, 
  184. but less flexible than the Durable Power of Attorney because it forces one to 
  185. anticipate every eventuality, rather than just appointing a decision-maker who can 
  186. make the decisions for you.

  188. Creating the Living Will is simple.  Just select Directives - Living Will, and follow the 
  189. instructions.  The witnesses should be unrelated, and not be health care 
  190. professionals or beneficiaries of your estate.  It is recommended that the 
  191. document be notarized, although this is not a requirement in some jurisdictions.

  193. When you have answered all of the questions, the Living Will will be printed.  If at 
  194. any time you would like to quit, just press Cancel.  If you start over by reselecting 
  195. Directives - Living Will, your original data will still be present.  However, when you 
  196. exit the program the data is erased.

  198. Just as a computerized diagnosis program is no substitute for a physician, this 
  199. program is no substitute for a lawyer.  While most jurisdictions will recognize the 
  200. Living Will created by Medical Consultant, it is still recommended that you have a 
  201. legal professional examine and approve the document.  Additionally, it is strongly 
  202. recommended that you discuss your Living Will with your friends, family and 
  203. physician in order that they are aware of it's existence and your wishes.  Give 
  204. your family a copy, as well as your physician(s).

  209. 5.  DURABLE POWER OF ATTORNEY
  210.  
  211. The Durable Power of Attorney for Health Care Decisions is a very powerful 
  212. document.  It enables you to designate an "agent" who will make medical 
  213. decisions for you when you are no longer able to.  This document supplements a 
  214. Living Will, and extends your surrogate decision-making power to situations which 
  215. you could not have anticipated.
  216.  
  217. To create a Durable Power of Attorney, just select Directives - Durable Power of 
  218. Attorney.  Then, follow the instructions.  Enter your name and address, as well as 
  219. that of your witnesses.  The witnesses should not be members of your family, 
  220. health care professionals or beneficiaries of your estate.  The document should 
  221. be notarized, although this may not be a requirement in your jurisdiction.

  223. To stop at any time, select Cancel.  When you restart by selecting Directives - 
  224. Durable Power of Attorney, your original data will be displayed.  Once you exit the 
  225. program, it is erased.  When you have answered all of the questions, the Durable 
  226. Power of Attorney will be printed.

  239. 6.  CARDIOVASCULAR RISK ANALYST
  240.  
  241. This program is a special bonus program, available only to registered users of the 
  242. Medical Consultant program.  For details about registering this software, see 
  243. section 2 of this documentation.

  245. The Cardiovascular Risk Analyst (CRA) is based on the extensive data collected 
  246. by the Framingham study.  It enables you to determine your risk of developing 
  247. heart disease or having a stroke, based on a few easily measured attributes.
  248.  
  249. To load the program, just place it in any directory on your hard drive, and make 
  250. sure that the VBRUN100.DLL file is in your \WINDOWS directory (or somewhere 
  251. on your path).  To run the program, select File-Run from the Program Manager 
  252. and type CV-RISK, or select the CV-RISK.EXE file from the File Manager.

  254. After an introductory screen, select Analyze.  You will be given a choice of Heart 
  255. Disease or Stroke.  Choose one.  Next, a form will be shown asking for some 
  256. information about your personal risk factors.  For details about a term or definition, 
  257. press the Help button.  To start over, press the Clear data button.  To select a 
  258. different analysis or quit, press the Cancel button.  Once you have entered all of
  259.  your data, press the Calculate button, and your risk analysis will be displayed.

  261. You are given the option of having a detailed report printed, which will tell you in 
  262. real English what your current risk is, how you can change it, and what happens 
  263. when you do change it.

  292. APPENDIX...
  293.  
  294. Windows is a complex environment, and conflicts are inevitable.  Kind of like a 
  295. family... the more brothers and sisters you have, the less likely you are to get 
  296. along with all of them.  Enough philosophy though...
  297.  
  298. 1. If you have difficulty printing, make sure that you have one of the following 
  299. fonts loaded:  Helv, Helvetica, Ariel or Modern.  If necessary, use the Windows 
  300. Control Panel and select Fonts - Add to load one of these fonts.
  301.  
  302. 2. If the program doesn t run at all, make sure:
  303.     -- that the VBRUN100.DLL file is in your path somewhere (usually in the
  304.     \WINDOWS directory)
  305.     -- that you have enough memory to hold about 400k of program (close 
  306.     unnecessary programs if possible)
